Measuring Health Outcomes: Success Measures Evaluation Tools for Community Development and Health

This new toolkit, developed by Success Measures at NeighborWorks America, offers more than 65 data collection tools to help community development professionals understand the effect housing practices have on resident and community health. These tools assess areas such as an individual’s health status, beliefs, attitudes and behaviors; personal and community factors; and access to services.

While it is increasingly clear that housing affects health, there remains much to learn about which policies and practices have impact, and in what ways. Better data collection is vital to building the body of evidence needed to develop, share and tailor interventions.

Developed by Success Measures, an evaluation resource group at NeighborWorks America, this new resource offers data collection tools that improve understanding of ways to address the social determinants of health. A new national initiative, jointly designed and implemented by NeighborWorks America and Enterprise Community Partners, is using the new tools to help demonstrate and document health outcomes in low- and moderate- income communities.
